Two registration cards and a $50.00 non-refundable registration fee are required when you register. Only one registration fee is required per family. After registration, children are registered and enrolled as we have openings throughout the calendar year.
PHYSICAL EXAMINATIONS
Each child is required, by the state of Illinois, to have a physical not more than six (6) months before entry. Additionally, a T.B. test, a lead assessment and screening are necessary and the following immunizations are required:
4 DPT 3 Polio 3 HB 1 MMR 4 HIB Chicken Pox Vaccine
A completed medical form must be on file before a child can begin school.
